Welcome to Rheumatology & Arthritis Center

New patient forms must be submitted online or mailed to our office prior to your appointment. If new patient forms are not received, you may not be seen, and your appointment may be rescheduled. If forms are submitted online, medications must be listed in a designated section.

Please bring your current insurance cards and a list of all medications to your appointment.

Your copayment/deductible is payable upon check-in; if you do not pay your copayment/deductible and our office has to bill you, there will be an additional charge.

Please inform our office of any insurance change/cancellation prior to your appointment; if the insurance provided is inactive or terminated, your appointment will be canceled until valid insurance is supplied to our office.

We accept cash, checks, MasterCard, Visa, Discover, and American Express.

What to Expect Upon Arrival at Our Office

When arriving at our office, you will be greeted with a warm welcome from the office staff. Our office is committed to providing patient-centered quality care. Our staff is very knowledgeable and up to date on treatment, medications, and management of all disease states treated.

Initial consultation varies for each patient; questions range from medical history to current symptoms to medications currently being taken.

Our doctor may order additional labs and x-rays to assist with an accurate diagnosis. Once all tests and history of symptoms are acquired, our doctor can provide a treatment plan that may include follow-up appointments and medications to help manage your disease.

About Our Office

Practice established in 2008, Board Certified Rheumatologist. Caring, experienced, and knowledgeable staff. Patient-centered care. Referral-based appointments. Most insurances are accepted.

Cancellation Policy

Appointment must be canceled 48 hours in advance or a $25.00 fee will be charged.

If two or more scheduled visits are missed, the patient can be discharged from the practice.
